You are not required to use this form. You may send in an email with the information requested on the form to the national office at asaylor@connect2amc.com.

The ASBH Nominating Committee solicits your nominations for candidates for the positions of Secretary, Directors-at-Large, and Nominating Committee. The candidates will take office at the 2010 Annual Meeting. A list of the 2009-10 Board of Directors and Nominating Committee is included for your reference.  To be eligible, a nominee must be an active voting member. As stated in the ASBH Bylaws, the Board of Directors has supervision, control and direction of the affairs of the Society, including the disbursement of funds, and determines its policies within the limits of the bylaws.

Secretary
The elected candidate will serve a two-year term and will take office at the 2010 Annual Meeting. The Secretary reviews the minutes from leadership meetings, is the Society's main communications liaison, and is a member of the Executive Committee. The Secretary participates in monthly leadership conference calls scheduled for 90 minutes each and two in-person Board of Directors meetings, one at the ASBH Annual Meeting and one in the Spring, usually held in April or May.

Director-at-Large
Nominations are sought for three Directors-at-Large to take office at the 2010 Annual Meeting. The elected Directors-at-Large will serve three-year terms. Directors participate in conference calls scheduled for 90 minutes every other month (or monthly if needed) and two in-person Board of Directors meetings, one at the ASBH Annual Meeting, and one in the Spring, usually held in April or May.

Nominating Committee
The Nominating Committee solicits nominations for the open Board and Nominating Committee positions for the following year, prepares and submits the slate of candidates to the Board for approval, and resolves any ties in the number of votes during an election. Nominations are sought for two Nominating Committee members to take office at the 2010 Annual Meeting. The Nominating Committee consists of six members, none of whom are Board Members. Nominating Committee members serve two-year staggered terms. Two members are elected by the membership, and one member is appointed by the Board each year.

Instructions
The Nominating Committee requests that all nominations be as complete as possible.  Complete the nomination form or prepare a letter of nomination, including the nominee's name, current position, areas of interest, ASBH involvement, professional activities and qualifications for each nominee. A brief biosketch (no more than 1 page in bullet format) is requested, though not mandatory. You may nominate one or more members for each position. (Duplicate the form as necessary.)  Self nominations are also welcomed.
